We act as the Data Controller for all personal information processed as part of our physiotherapy services.

- Details of neurological conditions

- Symptoms, assessments, clinical notes

Your data is stored securely in encrypted, password-protected systems in compliance with UK GDPR and HCPC/CSP requirements.

Access is restricted to those involved in your care.

Paper records, if used, are stored securely and destroyed appropriately.

 

6. Sharing your information

Your information is only shared:

- With your consent, for example with your GP, consultant, or MDT

- When legally required or where there is a serious risk of harm

 

We never share your data for marketing purposes.

 

7. How long your information is kept

Records are retained in line with professional guidance:

- Adults: 8 years after final contact

- Children: Until age 25 (or 26 if seen at 17)

- Longer if required for legal reasons

 

8. Your rights under UK GDPR

You have the right to:

- Access your data

- Request corrections

- Withdraw consent

- Request erasure where legally possible

- Restrict processing

- Request electronic copies (data portability)

 

9. Withdrawing consent

You may withdraw your consent at any time. This may affect your ability to continue receiving treatment, as physiotherapy requires processing of health data.

 

10. Community-Based Physiotherapy Work

The same data protection standards apply during home visits or community sessions. Information collected during these sessions is recorded securely in the same encrypted system used for all appointments.

Evolve Neuro Physiotherapy – Terms and Conditions

1. Introduction

These Terms and Conditions outline the basis on which Evolve Neuro Physiotherapy provides specialist neurological physiotherapy services. Please read them carefully before commencing treatment.

2. Provision of Physiotherapy Services

• All assessment and treatment sessions are provided by a physiotherapist registered with the Health and Care Professions Council (HCPC).

• Sessions may be delivered in your home, a community setting, or virtually.

• Session durations: Initial assessment up to 60 minutes; follow‑up sessions up to 45 minutes.

• Following assessment, an individualised treatment plan will be proposed. If physiotherapy is not appropriate, onward referral may be recommended.

• Treatment benefits, risks, and potential side effects will be discussed. You may withdraw consent at any time.

3. Community Visit Expectations

• You must ensure the treatment environment is safe, private, and appropriate for assessment and exercise.

• Pets should be secured where needed for safety.

• If the physiotherapist feels the environment is unsafe, the session may be stopped and the full fee will remain payable.

4. Report Writing

• Comprehensive written reports are available upon request and charged at £110 per hour.

5. Travel Costs

• Locations beyond a 10‑mile radius of Liphook (GU30 7TW) may incur additional travel charges, agreed in advance.

 

6. Liaison with Other Professionals

• Communication with healthcare professionals, case managers or insurers is charged at £55 per 30 minutes (minimum 30‑minute charge).

7. Stopping a Session

• You may request to stop treatment at any time.

• The physiotherapist may stop a session if reasonable instructions are not followed or if safety is compromised. The full session fee will apply.

8. Early Termination of Services

Services may be terminated in exceptional circumstances including illness, inappropriate behaviour, refusal to cooperate, or conflict of interest. Reasonable notice will be given where possible.

9. Fees

• Fees are agreed before treatment commences.

• You are responsible for all charges, including cancellation fees.

• Clients using insurance will be invoiced directly and must seek reimbursement themselves.

​

10. Fee Reviews

• Fees may be updated with a minimum of one full calendar month’s notice.

​

11. Payment Terms

• Invoices are issued after each session and payable within 14 days.

• Payment is accepted via cash, or  bank transfer.

​

12. Cancellation Policy

• Cancellations made within 24 hours of the appointment will incur a 50% session fee.

​

13. Confidentiality

• All personal information is treated confidentially and will only be shared with your consent, unless required by law or to prevent serious harm.

​

14. Data Protection

• Evolve Neuro Physiotherapy is compliant with UK GDPR.

 • Personal and clinical information is stored securely using encrypted systems.

• A full Privacy Policy is provided separately.

15. Chaperone Policy

• You may request a chaperone for any appointment. Please notify ahead of time if you wish one to be present.

16. Limitation of Liability

• Liability for loss or damage (excluding death or personal injury due to negligence) is limited to the total amount paid for services.

• We are not liable for indirect or consequential losses.

​

17. Indemnity

• You agree to indemnify Evolve Neuro Physiotherapy for loss, damage or legal costs resulting from your breach of these Terms and Conditions.

18. Variation

• These Terms and Conditions may be updated. Notice will be provided where appropriate.

​

19. Severance

• If any clause is found to be invalid or unenforceable, the remaining terms will continue in full force.

​

20. Events Beyond Our Control

• We are not liable for delays or service failures caused by events outside our control, including illness, technical failure, weather issues, or emergencies.

21. Communication

• Written notices may be sent to the business address available upon request.

• Communication will be sent to the contact details you provide on registration.

​

22. Governing Law

• These Terms and Conditions are governed by the laws of England and Wales. Disputes will fall under the jurisdiction of the English courts.
